We are seeking a highly skilled Advanced Heart Failure Specialist to join our team at Piedmont HealthCare. As a key member of our Advanced Heart Failure Center, you will provide comprehensive care to patients with advanced heart failure in both inpatient and outpatient settings.

Key Responsibilities:
  • Provide advanced heart failure care in both inpatient and outpatient settings
  • Collaborate with a multidisciplinary team to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans
  • Order and interpret diagnostic tests, including echocardiograms and cardiac catheterizations
  • Develop and implement patient education programs to promote self-management and improve outcomes
Requirements:
  • Master's degree in Nursing or related field
  • Current licensure as a Nurse Practitioner in the state of Georgia
  • At least 2 years of experience in advanced heart failure care
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
