POSITION SUMMARY: The role involves delivering operational assistance for new healthcare service locations aimed at enhancing the well-being of low-income women in marginalized areas by broadening access to preventive health services and support systems through established women's health initiatives.
SUPERVISION: Reports directly to the Program Manager.
PHYSICAL REQUIREMENTS: The position necessitates extended periods of sitting, bending, and stooping. May involve lifting items up to 25 pounds. Proficient use of office machinery, including computers, telephones, copiers, and scanners is required.
KEY RESPONSIBILITIES:
- Evaluates patients' obstacles to accessing women's preventive health services using a comprehensive assessment tool that identifies non-medical service needs.
- Informs patients about available services through standardized educational resources.
- Monitors patient follow-ups to ensure completion of necessary services.
- Gathers data to assess the primary outcomes of patient navigation, including adherence to service recommendations and tracking patients who are lost to follow-up.
- Maintains detailed records, including the number of patients served, referrals made, outcomes achieved, and other relevant activities.
- Establishes and sustains communication with patients to provide referrals and conduct follow-ups.
- Demonstrates the ability to work in diverse environments with culturally varied families and communities, ensuring cultural sensitivity and appropriateness.
- Continuously enhances knowledge of community needs, resources, services, and programs available.
- Upholds confidentiality in accordance with HIPAA regulations regarding patient information.
- Collaborates effectively as part of a team while exhibiting self-motivation and independence.
- Engages in all mandatory training and educational sessions as required by the program.
- Performs additional duties as assigned.
QUALIFICATIONS:
- Bachelor's degree in Social Work, Sociology, Psychology, Health Education, or a related field from an accredited institution.
- Relevant work experience in a healthcare setting may substitute for educational qualifications.
- Bilingual proficiency in English and Spanish is preferred.
- Possession of reliable transportation.
- Valid driver's license and appropriate liability insurance.
SKILLS AND COMPETENCIES:
- Ability to collaborate with individuals or groups to foster community education and client engagement.
- Effective communication skills in both English and Spanish.
- Capacity to interpret, comprehend, and execute instructions and directives.
- Ability to work autonomously as well as part of a team.
- Willingness to accept guidance and direction.
- Basic understanding of compliance and HIPAA regulations.
- Ability to cultivate and maintain positive working relationships with colleagues, visitors, and patients.
- Proficient in Microsoft Office Suite (Excel, Word, Publisher, and PowerPoint).
