Skyland Trail, a nonprofit mental health residential treatment program, is seeking a Licensed Practical Nurse to join our team.
Job SummaryThe Licensed Practical Nurse will be responsible for providing high-quality patient care, working collaboratively with our multidisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities- Assess, plan, implement, and evaluate patient care using the nursing process.
- Administer medications and monitor self-administration of medications at residential sites and the Health and Wellness Clinics.
- Perform medical and health screenings, collect specimens, and obtain vital signs and weights.
- Complete admissions evaluations, including obtaining necessary information for comprehensive nursing, medical, and psychiatric care.
- Assess signs and symptoms indicating physiological and/or psychosocial changes in patients' conditions.
- Follow up on emergency, clinic, and hospital visits.
- Document patients' responses to nursing and other interventions.
- Record and report observed symptoms, reactions, treatments, and changes in patients' conditions.
- Orient patients and families to the facility as needed, providing a safe environment with monitoring as appropriate.
- Monitor activities of visitors and perform room searches as necessary.
- Instruct patients in activities of daily living, interpersonal, and socialization skills.
- Assist in crisis situations in coordination with the crisis communication plan.
- Intervene as necessary to decrease panic of patients.
- Facilitate an educational group per week for patients.
- Participate in change of shift reports and ensure medication management policies are followed.
- Facilitate/co-facilitate community meetings as needed.
- Serve as the alternate residential representative to the Safety and/or QI Committee if needed.
- Participate in Infection Control and Medication Management Policy and Procedures reviews as needed.
- Perform other duties as assigned within the scope of this Nurse's licensure as per the Georgia Nurse Practice Act and Skyland Trail's policy and procedure.
- Maintain and stock inventory of office and/or clinical supplies as appropriate to the position.
- A current Practical Nursing Licensure for Georgia without restrictions.
- Minimum of two years' experience.
- Current physical and PPD.
- Two years psychiatric nursing experience preferred.
- Unrestricted mobility and ability to physically support patients in the event of urgent or emergent medical issues.
- Current CPR and First Aid certifications.

- Knowledge of emergency and safety procedures.
