Job Summary

We are seeking a highly skilled and experienced Licensed Practical Nurse to join our team at the Fountainview Center LP. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Supervise and evaluate the work of nursing assistants and other support staff to ensure that patient care is delivered in a safe and effective manner.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals to ensure that patient care is coordinated and comprehensive.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited LPN educational program and licensed in the state of Georgia.
  • At least 2 years of experience in long-term care or a related field.
  • Strong leadership and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Basic computer skills and proficiency in electronic medical records.
