About the Role

Curo Health Services is seeking a licensed Master's level Social Worker to join our interdisciplinary team. As a Social Worker, you will play a vital role in providing psychosocial support to patients and their families, ensuring their needs are met and their goals are achieved.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient and family needs, goals, and interventions.
  • Develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on end-of-life care, advance care planning, and bereavement.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school.
  • Licensure as a Social Worker in the state where the hospice is located.
  • At least one year of experience as a Social Worker in a healthcare or hospice setting, with three to five years preferred.
  • Knowledge of terminally ill patients and their families, as well as understanding of hospice and the psychosocial dynamics of illness, loss, and death.
