Key Responsibilities- Conduct comprehensive psychosocial assessments to identify patient and family needs, goals, and interventions.
- Develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients and their families.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families on end-of-life care, advance care planning, and bereavement.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Master's Degree in Social Work from a CSWE-accredited school.
- Licensure as a Social Worker in the state where the hospice is located.
- At least one year of experience as a Social Worker in a healthcare or hospice setting, with three to five years preferred.
- Knowledge of terminally ill patients and their families, as well as understanding of hospice and the psychosocial dynamics of illness, loss, and death.
