- Provide social work services in accordance with the plan of care, ensuring timely communication of changes in patient status to other healthcare team members and care center office staff.
- Educate patients, family members, and other team members on medical social work interventions and treatment plans.
- Perform care planning, transfer, and discharge functions, identifying risks or threats to patient health in the patient's environment and implementing appropriate interventions.
- Holds a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
