Job Title: Health and Wellness Director

Cogir Senior Living is seeking an experienced and compassionate RN Health and Wellness Director to join our team at Cogir of Tigard. As a key member of our management team, you will be responsible for ensuring the health and wellness of our residents, performing initial and ongoing assessments, and coordinating with other departments to provide the highest quality of care.

Key Responsibilities:
  • Foster a resident-centered approach to planning and delivering services, supporting residents' rights to participate in decision-making and treating residents with dignity and respect.
  • Assess residents' health, functional, and psychosocial status, initiating individualized service plans, and proactively managing care and services for each resident.
  • Establish and maintain the medication program, ensuring compliance with company, federal, state, and local requirements.
  • Oversee all hiring, promotions, disciplinary actions, and termination of employment of licensed nursing staff, Med Techs/CMA/CNAs, and all Care staff.
  • Provide hands-on training, supervision, and staff development, ensuring a positive and energetic atmosphere within the community.
  • Communicate regularly with residents' families regarding health care needs, maintaining current departmental policies, procedures, and licenses.
  • Ensure resident records and charting are current and up to date, participating in marketing efforts by representing the community positively to outside agencies.
Requirements:
  • Positive team player mentality and passion for serving seniors.
  • RN nursing license in good standing.
  • Minimum of 4 years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
  • Minimum of 2 years in a supervisory/management role, with experience in budgets, staff development, training, and scheduling.
  • An associate degree or higher is required; additional coursework in management is an asset.
  • Experience in Memory Care is preferred.
  • Outstanding organizational skills and proficiency in Microsoft suite.
  • Maintains current state-required certifications, like First Aid, CPR, and others.
  • Ability to work a flexible schedule, including some weekends, holidays, and overtime, to meet the position's requirements.
  • Capacity to be on-call 24/7.
  • Meet all mandatory health requirements according to state regulations.
  • Has compassion and a special interest in working with the senior population and individuals living with memory loss.
