Job Overview

We are seeking a skilled Social Care Specialist to join our team. The ideal candidate will have a strong background in mental health and a passion for delivering exceptional care to our patients.

The successful candidate will be responsible for evaluating and supporting patient needs, providing integrated treatment planning, delivering behavioral health services, and coordinating care with the patient's primary care provider and other behavioral health providers.

We offer a competitive salary range of $50-$75 per hour for LMSW and $75-$125 per hour for LCSW, commensurate with experience. Our comprehensive benefits package includes Medical, Dental, and Vision Insurance with no employee contribution, as well as a team appreciation bonus, paid time off, and 403(b) retirement plan.
