Job Title: Home Health Speech Therapist

We are seeking an experienced Home Health Speech Language Pathologist to join our dynamic team. As a Home Health Speech Therapist, you will provide speech therapy services to patients in their homes, according to a written physician's plan of care.

Responsibilities:
  • Assess and evaluate rehabilitative/functional status, and participate in the development of the total plan of care.
  • Provide a full range of Speech-Language Pathology Services as ordered by a physician.
  • Improve or maximize the communication of the patient.
  • Assist/develop the plan of care and review as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for patients.
  • Prepare and submit a clinical progress summary based on the attainment of goals as directed by Organization policy.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks.

Requirements:
  • Master's degree from a Speech-Language Pathology program certified by the American Speech Association to practice in the state of Michigan.
  • License and certification as a Speech-Language Pathology, issued by the Michigan state.
  • Minimum of one-year healthcare experience.
