Job Summary

We are seeking an experienced Speech Language Pathologist to provide high-quality speech therapy services to patients in their homes. As a Speech Language Pathologist with Hope At Home Health Care, you will have the opportunity to work with a dedicated and passionate team to inspire, innovate, and influence the lives of our patients.

Responsibilities
  • Provide speech therapy services to patients according to a written physician's plan of care, including assessing and evaluating rehabilitative/functional status and participating in the development of the total plan of care.
  • Conduct home environment evaluations and provide full-range speech-language pathology services as ordered by a physician.
  • Improve or maximize patient communication and assist in the development and review of the plan of care as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals and participate in discharge planning for patients.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.

Requirements
  • Master's degree from a Speech-Language Pathology program certified by the American Speech Association to practice in the state of Michigan.
  • License and certification as a Speech-Language Pathology, issued by the Michigan state.
  • Minimum of one-year healthcare experience.
