Nest Health is seeking a highly skilled and compassionate Family Nurse Practitioner/Physician Assistant to join our team as an In-Home Care Specialist. As a key member of our care team, you will provide comprehensive primary care to patients of all ages and backgrounds, both in the home setting and virtually.
Key Responsibilities- Conduct physical exams, screenings, and lab tests, overseeing the health of families under physician supervision
- Collaborate with specialists in nutrition and behavioral health to provide holistic care
- Deliver culturally congruent, non-judgmental care, and work closely with Medical Assistants trained as Community Health Workers
- Provide urgent care, handle acute medical visits and calls, and prevent emergency department visits or hospitalizations
- Maintain clinical policies and standards, patient safety, documentation, and SOAP notes
- Order, administer, and record diagnostic tests, diagnose health conditions, and assess cases presented by the care team
- Develop treatment plans, propose treatments for chronic and infectious diseases, and educate patients about their conditions
- Completion of Physician Assistant (PA) or Nurse Practitioner (NP) accredited program and ability to obtain an active, unrestricted Louisiana license
- Experience and training in family medicine and the ability to treat patients from infancy to 64 years
- Availability: 1-2 days a week, with Saturday availability preferred
- Proficiency in and experience using electronic medical records (EMRs)
- Maintains current CPR certification (BLS)
- Able to meet Nest Health credentialing requirements
- Malpractice profile meets Nest Health risk guidelines
