Job Description

We are seeking a highly skilled Patient Navigator to join our team. As a Patient Navigator, you will be responsible for ensuring that patients receive the highest level of care before, during, and after their surgical procedure.

Key Responsibilities:

  • Contact patients prior to surgery to review medical history and verify accuracy of documentation.
  • Assess patient's home and social situation to ensure optimal post-operative care.
  • Provide surgeon-specific education to patients regarding physical therapy, anticoagulation protocols, and other essential information.
  • Make follow-up calls to patients post-op to address concerns and facilitate appointments.

Qualifications:

  • Bachelor's degree in Nursing (BSN) required.
  • Minimum 5 years of experience in an orthopedic practice.
  • Clinical and back-office experience preferred.
