**Physical Therapy Assistant Opportunity at Creative Solutions in Healthcare - East**
We are seeking a skilled Physical Therapy Assistant to join our team at Dogwood Trails Manor, a reputable healthcare facility. This is an exciting opportunity for a motivated and compassionate professional to work with our residents and contribute to their recovery and well-being.
As a Physical Therapy Assistant, you will be responsible for administering medically prescribed physical therapy treatments, providing skilled services under the supervision of a Physical Therapist, and collaborating with our interdisciplinary team to achieve positive outcomes.
**Key Responsibilities:**
- Administering physical therapy treatments to relieve pain, improve or restore function, and promote healing.
- Providing skilled services/interventions in accordance with physician orders under PT supervision.
- Responding to requests for service by relaying information and referrals to Physical Therapist.
- Implementing and modifying patient's physical therapy plan of treatment with PT supervision.
- Measuring & recording patient's motor function, strength, and muscle performance.
- Monitoring patient response to treatment intervention and modifying treatment approach and directive as clinically appropriate with IDT collaboration.
- Providing clinical support and education to patients, family members, and caregivers.
- Ensuring documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
**Requirements and Skills:**
- Active/Valid Texas license as Physical Therapy Assistant.
- In good standing with all regulatory agencies and licensing boards.
- SNF/Long term care experience preferred.
- Current knowledge of treatment practices.
- Knowledge of Net Health Electronic Documentation System preferred.
