Occupational Therapist

We are seeking a skilled Occupational Therapist to join our in-house team at Dogwood Trails Manor, where we provide person-centered care to achieve positive outcomes for our residents.

This is a consistent Per Diem (PRN) position, offering flexible scheduling and the opportunity to make a meaningful difference in the lives of our patients.

The ideal candidate will have experience working in Skilled Nursing Facilities (SNFs) or Long Term Care settings and be knowledgeable about treatment practices. Familiarity with Net Health Electronic Documentation System is a plus.

• Conduct comprehensive evaluations to identify patient needs and develop personalized treatment plans.
• Collaborate with patients, family members, and caregivers to ensure effective care delivery.
• Provide recommendations for home modifications and adaptive equipment to enhance patient safety and independence.
• Ensure documentation meets regulatory requirements and company standards.

• Active/Valid Texas license in Occupational Therapy.
• In good standing with all regulatory agencies and licensing boards.
