As a Social Services Specialist within our hospice program, you will be instrumental in delivering psychosocial support and various services to patients and their families. Operating in inpatient, home care, and nursing home environments, you will formulate care plans, establish goals, and make recommendations tailored to patient needs. You will work collaboratively with interdisciplinary teams and act as a liaison between hospice personnel and other healthcare providers.

Key Responsibilities:

  • Deliver psychosocial support to hospice patients and their families
  • Engage in hospice interdisciplinary group meetings and collaborate with inpatient and nursing home staff
  • Create care plans and discharge strategies for patients
  • Ensure precise documentation of clinical data

Requirements:

  • Graduation from an accredited School of Social Work
  • At least one year of experience in healthcare social work
  • New York State MSW licensure
  • Proficiency in Microsoft Word and Excel
  • Valid New York State Driver's license and access to a vehicle
  • Strong ability to function effectively within a team environment
