Hospice Customer Service Representative

As a Hospice On-Call Customer Service Representative, you will serve as a liaison between Hospice Home Care Planners, Social Workers, and RN Case Managers and RN on-call Triage Nurses. You will respond to Social Work, RN Case Manager, patient, and customer complaints and grievances, taking accurate messages, problem-solving where applicable, and forwarding messages to the appropriate party as directed. You will also enter patient referral information into the Allscripts system, including patient demographics, diagnosis, and plan of care, and send tasks to Team Assistants for Insurance Verification and to vendor services for DME and HHA. Additionally, you will verify that all new physicians' license numbers, NPI numbers are registered, and check for duplicate physician entries before entering into Allscripts. You will contact Physicians, Clinics, Hospitals, Nursing Homes, and sub-acute facilities to develop, maintain, and follow-up on pending and/or new referrals. This full-time position has a schedule of 5pm to 1am.
Responsibilities:
• Respond to Social Work, RN Case Manager, patient, and customer complaints and grievances

Requirements:
• High school diploma or GED required
• Minimum 6 months call center experience
• Intake experience preferred
• Microsoft Word and Excel required
• STAT, Omni/MISYS preferred
• Strong verbal communication skills
• Ability to multi-task and work in a fast-paced environment
