RN Clinical Coordinator SNF Extended Care Full Time Days

Job Summary- Lead by example, promoting excellence in resident care and modeling professional demeanor.
- Ensure that self and supervised staff apply nursing principles, techniques, and practices according to HDMC policies and procedures.
- Function as the primary resource at the assigned unit, managing patient flow, residents' complaints, employee concerns, and serving as the primary contact for all physicians within the facility.
- Supervise nursing staff, assist with hiring, education, and disciplinary action of nursing staff, and ensure staff understanding and compliance with policies, procedures, and standards.
- Knowledge of HDMC policy/procedure, nursing certification requirements for own and others' positions.
- Perform own job duties and ensure that supervised staff perform their duties within legal and professional regulations and guidelines.
- Ensure that self and those supervised apply nursing principles, techniques, and practices according to HDMC policies and procedures.
- Lead by example to promote excellence in resident care.
- Model professional demeanor and act as a mentor.
- Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations.
- Observe Residents Bill of Rights and report incidents of elder abuse, if indicated.
- Ensure supervised staff also is aware of and follow these regulations.
- Arrive and leave work timely.
- Follow HDMC Human Resources policies, especially regarding dressing code, call-off policy, breaks, lateness, and sleeping while on duty.
- Ensure staffing for own shift and the following are adequate and appropriate according to set staffing guidelines.
- Contacts other nursing departments before calling in or calling off department staff.
- Appropriately and fairly sets staff assignments for their shift, and the following shift.
- Ensure staff workload is reasonable and as equally divided as possible.
- Function as the primary resource at the assigned unit, manages patient flow, residents' complaints, employee concerns, and is the primary contact for all physicians within the facility.
- Answers facility phone calls timely, responds to call lights via the call light system, and delegates when appropriate.
- Demonstrates ability to delegate and achieve results through others.
- Demonstrates understanding of and ensures compliance with all State and Federal requirements.
- Assumes leadership role for daily operations and takes appropriate action.
- Rounds on residents, nursing staff, and families daily to ensure good outcomes.
- Supervises nursing staff, assists with hiring, education, and disciplinary action of nursing staff.
- Assists with writing and revising, and ensures staff understanding and compliance with policies, procedures, and standards.
- Demonstrates the ability to coordinate response to emergencies in a calm and efficient manner.
- Understands and demonstrates ability to work within the departmental budget.
- Ensures staff schedule is accurate and timely, and checks staffing needs daily to provide that minimum ppd hour are met.
- Meets regularly with the medical director and other staff physicians to ensure proper communication is maintained.
- Ensures that communication with the Acute Hospital is accurate and cooperative.
- Functions as a floor nurse, and/or certified nursing assistant when the facility needs so warrant.
- Completes all daily, weekly, and monthly assignments as assigned.
- Completes other duties as assigned.
- Meets at least weekly with the DON.
- Associate Degree
- Current California Registered Nurse License (RN)
- BLS Certification
- 2 years experience as an RN/LVN
- Previous Subacute/ventilator experience preferred
- Contact with residents/patients under a wide variety of circumstances
- May be exposed to infections and contagious diseases
- Regularly exposed to the risk of blood-borne diseases
$77.02 hourly
**Individual wages are determined based upon a number of factors including, but not limited to, an individual's qualifications and experience.
