Job Title: RN Clinical Coordinator SNF Extended Care Full Time Days

Job Summary
  • Lead by example, promoting excellence in resident care and modeling professional demeanor.
  • Ensure that self and supervised staff apply nursing principles, techniques, and practices according to HDMC policies and procedures.
  • Function as the primary resource at the assigned unit, managing patient flow, residents' complaints, employee concerns, and serving as the primary contact for all physicians within the facility.
  • Supervise nursing staff, assist with hiring, education, and disciplinary action of nursing staff, and ensure staff understanding and compliance with policies, procedures, and standards.
Responsibilities
  1. Knowledge of HDMC policy/procedure, nursing certification requirements for own and others' positions.
  2. Perform own job duties and ensure that supervised staff perform their duties within legal and professional regulations and guidelines.
  3. Ensure that self and those supervised apply nursing principles, techniques, and practices according to HDMC policies and procedures.
  4. Lead by example to promote excellence in resident care.
  5. Model professional demeanor and act as a mentor.
  6. Knowledge of Residents Bill of Rights and Elder Abuse reporting regulations.
  7. Observe Residents Bill of Rights and report incidents of elder abuse, if indicated.
  8. Ensure supervised staff also is aware of and follow these regulations.
  9. Arrive and leave work timely.
  10. Follow HDMC Human Resources policies, especially regarding dressing code, call-off policy, breaks, lateness, and sleeping while on duty.
  11. Ensure staffing for own shift and the following are adequate and appropriate according to set staffing guidelines.
  12. Contacts other nursing departments before calling in or calling off department staff.
  13. Appropriately and fairly sets staff assignments for their shift, and the following shift.
  14. Ensure staff workload is reasonable and as equally divided as possible.
  15. Function as the primary resource at the assigned unit, manages patient flow, residents' complaints, employee concerns, and is the primary contact for all physicians within the facility.
  16. Answers facility phone calls timely, responds to call lights via the call light system, and delegates when appropriate.
  17. Demonstrates ability to delegate and achieve results through others.
  18. Demonstrates understanding of and ensures compliance with all State and Federal requirements.
  19. Assumes leadership role for daily operations and takes appropriate action.
  20. Rounds on residents, nursing staff, and families daily to ensure good outcomes.
  21. Supervises nursing staff, assists with hiring, education, and disciplinary action of nursing staff.
  22. Assists with writing and revising, and ensures staff understanding and compliance with policies, procedures, and standards.
  23. Demonstrates the ability to coordinate response to emergencies in a calm and efficient manner.
  24. Understands and demonstrates ability to work within the departmental budget.
  25. Ensures staff schedule is accurate and timely, and checks staffing needs daily to provide that minimum ppd hour are met.
  26. Meets regularly with the medical director and other staff physicians to ensure proper communication is maintained.
  27. Ensures that communication with the Acute Hospital is accurate and cooperative.
  28. Functions as a floor nurse, and/or certified nursing assistant when the facility needs so warrant.
  29. Completes all daily, weekly, and monthly assignments as assigned.
  30. Completes other duties as assigned.
  31. Meets at least weekly with the DON.
Requirements
  • Associate Degree
  • Current California Registered Nurse License (RN)
  • BLS Certification
  • 2 years experience as an RN/LVN
  • Previous Subacute/ventilator experience preferred
Working Conditions
  • Contact with residents/patients under a wide variety of circumstances
  • May be exposed to infections and contagious diseases
  • Regularly exposed to the risk of blood-borne diseases
Pay and Benefits

$77.02 hourly

**Individual wages are determined based upon a number of factors including, but not limited to, an individual's qualifications and experience.
