Hospice Care Assistant

We are seeking a compassionate and skilled Hospice Care Assistant to join our team at Northwell Health. As a key member of our hospice care team, you will provide high-quality patient care and support to individuals in need.
Key Responsibilities- Assist primary care nurses with patient care and develop a plan of care in accordance with the patient's needs and goals.
- Provide patient care in accordance with the plan of care, including activities of daily living, such as bathing, grooming, and oral hygiene.
- Perform light housekeeping tasks to maintain a clean and safe environment for patients.
- Document patient care and activities in accordance with agency policies and procedures.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- High School Diploma or equivalent required.
- Certification by New York State Department of Health approved home health aide training program required.
- Minimum one year experience in hospice, homecare, and/or nursing home setting preferred.
