Role Summary

As a vital member of our multidisciplinary healthcare team, the Social Worker, PRN will engage in case management, crisis intervention, assessment, counseling, and treatment planning tailored to the unique needs of patients and their families. This role requires a comprehensive understanding of community resources and evidence-based practices to effectively address the psychosocial and emotional challenges faced by our patients.

Key Responsibilities
  1. Conduct thorough psychosocial evaluations, utilizing screening tools to formulate appropriate care plans.
  2. Independently develop and implement safe and timely care or discharge plans in collaboration with patients, families, and the healthcare team.
  3. Provide crisis intervention and utilize advanced social work techniques to deliver trauma-informed care, focusing on social determinants of health.
  4. Assess the effectiveness of interventions and contribute to performance improvement initiatives within the organization.
  5. Share and maintain an updated directory of community resources, demonstrating creativity in resource identification for patients and families.
  6. Stay informed about relevant medical, cultural, and legal issues impacting patient care, ensuring proper documentation and communication of referrals.
  7. Facilitate individual and group counseling sessions using brief therapy models to support patients' mental health goals.
  8. Act as a liaison among patients, families, and various departments to enhance care coordination and maintain confidentiality in accordance with HIPAA regulations.
  9. Conduct educational sessions and serve as a resource for junior social workers within the department.
  10. Identify systemic barriers affecting patient health and access to care, advocating for equitable solutions within the healthcare system.
  11. Perform administrative duties as required and complete all assigned tasks as directed by supervisors.
Qualifications

Education and Experience
Master's degree in Social Work from an accredited institution and a minimum of an LMSW license from the Maryland Board of Social Work Examiners. A minimum of three years of relevant experience is required.

  1. Proficient in clinical social work practices, including assessment, planning, intervention, and evaluation.
  2. Strong communication skills, both written and verbal, with the ability to engage effectively with diverse stakeholders.
  3. Ability to adapt to dynamic environments and make informed decisions aligned with ethical standards.
  4. Demonstrated leadership qualities and innovative thinking in addressing complex patient needs.
  5. Commitment to upholding the principles and standards of social work practice as outlined by the NASW Code of Ethics.
