Job Summary

We are seeking a highly skilled and compassionate Medical Social Worker to join our team at Amedisys. As a Medical Social Worker, you will play a vital role in providing social work services to patients and their families, helping them to cope with health-related financial, social and emotional concerns.

Responsibilities
  • Assist patients and families with health-related financial, social and emotional concerns and assist with effective coping skills.
  • Provide social work services in accordance with the plan of care.
  • Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
  • Educate patients and family members and other team members on medical social work interventions and treatment plans.
  • Perform care planning, transfer and discharge functions.
  • Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks.
Qualifications
  • Holds a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
  • Has one year of social work experience in a healthcare setting.
  • Meets state and federal requirements for this position.
  • Current CPR certification.
