Job Title: Home Health & Hospice Licensed Practical Nurse (LPN)

About the Role:

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Home Health & Hospice team. As a Home Health & Hospice LPN, you will play a crucial role in providing exceptional care to patients in their homes. Your primary responsibility will be to verify and execute the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.

Key Responsibilities:

  • Provide essential nursing treatments, promptly addressing patient pain and assisting with activities of daily living.
  • Educate patients and caregivers on self-care techniques.
  • Administer medications, ensure the availability of necessary supplies, and maintain accurate patient records.
  • Liaise with supervising RNs or physicians on patient assessments and any changes in conditions.
  • Contribute to team meetings to ensure cohesive patient care coordination.

Requirements:

  • Current Licensed Practical Nursing License in the state of practice.
  • 1+ years of Home Health & Hospice experience preferred.
  • Ability to work independently, be self-directed, and flexible.
  • Ability to use a full range of motion and lift 35lbs.
  • Effective verbal and written communication and time management skills.
  • Valid Driver's License (clean record) and reliable transportation.
  • Proof of current auto insurance and registration.
  • Ability to pass a Criminal Background Check.
  • Valid AZ Fingerprint Clearance Card (we can help).
  • Current CPR Certification (we can help) and Negative TB Test (we can test).
