HarmonyCares is seeking a skilled Nurse Practitioner to provide in-home health risk assessments and transitional care. The ideal candidate will have current RN and NP licensure, CPR certification, and enrollment in Medicare & Medicaid.
Key Responsibilities:
Perform comprehensive in-home health risk assessments to identify patient needs and develop personalized care plans. Educate and counsel patients and their families on health management and disease prevention. Conduct follow-up visits to ensure seamless transitional care and optimal patient outcomes.
