Job Description:

We are seeking a skilled Care Coordinator to join our team at Humana. As a Field Service Coordinator, you will play a vital role in helping us put health first for our customers.

About the Role:

This is a full-time position that requires a high level of travel, with a minimum of 100% coverage across 6-7 counties in Indiana. You will be responsible for meeting with members in their locations, assessing their needs, and connecting them with quality services to promote their well-being.

Responsibilities:

• Administer initial and ongoing long-term services and support (LTSS) related assessments through person-centered thinking approaches.
• Contact members telephonically and/or in-person to establish goals and priorities, evaluate resources, develop plan of care, and identify LTSS providers and community partnerships.
• Develop and continuously modify service plans, involving applicable members of the care team.
• Support members through navigation of their LTSS and related environmental and social needs.
• Foster relationships with healthcare providers, community agencies, and other stakeholders to ensure seamless care coordination.
• Maintain accurate records and documentation, adhering to HIPAA guidelines.

Requirements:

• Prior experience in healthcare and/or case management.
• Intermediate to advanced computer skills, including Microsoft Office Suite.
• Exceptional communication and interpersonal skills, with the ability to build rapport with internal and external customers and stakeholders.
• Critical thinking, organizational, written, and verbal communication skills, with the ability to problem-solve effectively.
• Able to manage multiple priorities in a fast-paced environment, working independently with minimal supervision.
