Job Summary

We are seeking a skilled Physical Therapist to join our team at Pathwell Health. As a Physical Therapist, you will play a vital role in helping our patients achieve optimal mobility, relieve pain, and improve their overall quality of life.

Key Responsibilities
  • Develop and implement personalized rehabilitation plans for patients in their homes
  • Conduct thorough assessments to identify patients' physical strengths and limitations
  • Educate patients and their families on exercise programs and therapeutic procedures to promote continued progress
  • Accurately document patient observations, interventions, and evaluations
  • Collaborate with physicians and other healthcare professionals to ensure comprehensive care
  • Participate in educational activities and contribute to the growth and development of our team
Requirements
  • Graduation from an approved Physical Therapy curriculum
  • Current and valid Connecticut Physical Therapist license
  • Recent work experience in home health (preferred)
  • Strong clinical assessment, organizational, and communication skills
  • Ability to work well under pressure and maintain a calm and focused demeanor in emergent situations
Preferred Qualifications
  • Bilingual (Spanish) proficiency
  • Strong interpersonal skills and good judgment
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping and documentation
