About the Role
Nuvance Health is seeking a skilled Physical Therapist Assistant to join our home healthcare team. As a Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to provide high-quality patient care in the comfort of patients' own homes.
Responsibilities
- Work collaboratively with a licensed Physical Therapist to develop and implement individualized treatment plans for patients.
- Perform physical therapy procedures and related tasks under the supervision of the Physical Therapist.
- Provide patient and family education and training on physical therapy techniques and exercises.
- Document patient visits and complete documentation within 24 hours of visit, adhering to agency guidelines.
- Participate in Performance Improvement activities and peer review.
- Complete continuing education requirements as per agency and state licensing regulations.
- Perform other duties as assigned by the Physical Therapist or management.
Requirements
- Customer relation skills with patients and staff.
- All PT modalities, use of a computer.
- Minimum experience: zero to six months.
- FIM certification desired.
Working Conditions
- Significant manual skills/motor coordination and finger dexterity.
- Some occupational risk.
- Extremely heavy effort, may exert > 125 lbs. of force.
- Some exposure to dirt, odors, noise, human waste, etc.
