Key Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs.
  • Provide one-on-one physical therapy sessions to patients in their homes, focusing on improving mobility, strength, and function.
  • Collaborate with other healthcare professionals, such as nurses and doctors, to ensure comprehensive care and coordination.
  • Monitor patients' progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and caregivers to educate them on treatment plans and provide support.
Requirements
  • Current licensure to practice as a physical therapist within the state of operation.
  • Minimum nine months of experience as a physical therapist.
  • Preferred experience in community or home health settings.
  • Master's degree in physical therapy from a CAPTE-accredited program.
  • Valid CPR certification.
  • Licensed driver with a reliable vehicle for travel to business locations.
