Job Title: Physical Therapist - Home Health Care Specialist

Job Summary:

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Care Specialist. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients in their homes, providing rehabilitative care and skilled nursing services.

Responsibilities:

  • Conduct home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements to assess client needs.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations related to client care management and services provided.

Qualifications:

  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) are preferred.
