Job Summary

We are seeking a compassionate and skilled LPN to join our hospice care team as a Patient Care Coordinator. In this role, you will collaborate with our team to provide management and delivery of patient care plans, ensuring the highest level of care and support for our patients and their families.

Responsibilities
  • Collaborate with the care team to develop and implement patient care plans
  • Provide direct patient care, including medication administration and wound care
  • Conduct regular assessments and evaluations to ensure patient needs are met
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • LPN license in the state of employment
  • Graduate from an approved school of practical nursing
  • Two years of LPN experience, with hospice experience preferred
  • Current driver's license and ability to spend approximately 20% of work time driving to/from patient locations
  • Ability to self-manage time and schedule, with a focus on meeting critical deadlines for charting
  • Demonstrated experience with tablets, mobile phones, and EMR software
