Primary Care Team Coordinator

The Staff Pad is seeking a highly skilled and detail-oriented Primary Care Team Coordinator to join our team.
About the JobThis is a dynamic role that requires strong organizational skills, effective communication, and a passion for delivering exceptional patient care.
As a Primary Care Team Coordinator, you will play a vital part in providing comprehensive primary care to all patients. You will work closely with our multidisciplinary team to ensure seamless delivery of care and support.
Responsibilities include preparing for and attending team meetings, identifying clinical needs, collaborating on developing team priorities and patient goals, managing referrals, updating health maintenance records, and more.
We are looking for someone with 2 years of clinic experience and current Montana licensure as a Licensed Practical Nurse. Basic Life Support (CPR) certification and re-certification every two years are also required.

- A competitive salary of $45,000 - $55,000 per year based on location and experience.
