Key Responsibilities:

  • Provide exceptional customer service by coordinating patient referrals for home care services
  • Assist with the intake process, including completing forms, coding, and routing documents for admission approval
  • Conduct initial nursing insurance authorization on new referrals and maintain a tracking log for insurance authorizations
  • Communicate with staff and community resources to develop a positive rapport and ensure seamless care coordination
  • Perform other duties as assigned by the Intake Manager/Administrator

Requirements:

  • LPN license in the State of Virginia (preferred)
  • Experience in home health care or intake coordination
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
