Job Title: Food Program Director

Fair Haven Community Health Care is seeking a highly motivated and experienced Food Program Director to lead our food-related programs. As a key member of our leadership team, you will be responsible for growing and sustaining our food programs, which aim to support our patients' nutritional needs and promote healthy lifestyles.

Job Summary:

The Food Program Director will oversee the implementation and operation of our 'Food as Medicine program', which includes an on-site food farmacy, prescription-for-food program, and nutrition education. You will work closely with medical providers, primary care teams, care coordination, and social services agencies to provide nutrition education and support to families with chronic disease/malnutrition and food insecurity.

Key Responsibilities:
  • Design and implement food programming to meet the needs of our patients, including an on-site Food Pharmacy
  • Work with the Population Health Team to identify target patient populations for the 'Prescription for Food' intervention
  • Recruit, train, and supervise interns and volunteers for various food pharmacy responsibilities
  • Coordinate community-facing nutrition education programming in our multipurpose room/kitchen space
  • Stock and maintain pharmacy shelves in coordination with the food pantry, ensuring nutritional content of foods
  • Maintain statistics on food pharmacy visits and items procured for FHCHC and community food bank
  • Participate in budget development, track expenses, and ensure budget compliance
  • Participate in grant writing and maintain grants, including completion of outcome reports
  • Coordinate various licenses and agreements with Connecticut food banks and the Connecticut Department of Health
Requirements:
  • Bachelor's degree in Nutrition or related field required; Master's degree in Nutrition or related field preferred
  • Registered Dietitian certificate; CT State Board licensure
  • Experience with one-on-one and group nutrition counseling and program implementation and management
  • Cultural competence and sensitivity to work with a diverse population; fluency in Spanish language and experience implementing and scaling programs, managing teams, and providing culturally competent nutrition programming is highly desired
Working Conditions:

This is a Category I—Increased Risk Position, with potential exposure to hazardous waste and blood-borne pathogens in clinical settings. The ideal candidate must be able to use computer software systems, including word processing, spreadsheet, presentation, and nutrient analysis programs, as well as varied electronic medical records systems.
