About the Job

This Registered Nurse Case Manager role offers an exciting opportunity to join our team at ABLE, Inc. and make a meaningful impact in the lives of individuals with developmental disabilities.

As a key member of our team, you will be responsible for delivering comprehensive healthcare services, coordinating services and supports, and ensuring the well-being of our clients.

Responsibilities and Requirements

  • Case Management:
    • Conduct comprehensive assessments and develop individualized care plans that address unique health needs.
    • Coordinate services and supports to ensure seamless delivery of care.
    • Monitor progress and revise plans as needed to facilitate opportunities for growth and independence.
  • Health Supports:
    • Monitor physical and mental health needs, including medication management and appointments.
    • Ensure adequate environmental support and assistive devices.
    • Provide training and instructions to staff on nursing tasks.
  • Staff Training and Development:
    • Provide training to direct care staff on nursing tasks and competency in delegated responsibilities.

Qualifications and Benefits

  • Bachelor's Degree and Registered Nurse license in North Dakota.
  • Valid driver's license.
  • Proficiency in computer systems, including Outlook, Teams, and Therap.
  • A competitive salary range of $ $72,000 - $85,000 per year , depending on experience.
