Job Description

Job Summary:

Patriot At Home is a rapidly growing, quality-focused, family-owned and operated local home health network seeking a Registered Nurse Case Manager to join our team. As a valued member of our Patriot Family, you will provide nursing care in accordance with the patient's plan of care, including comprehensive health and psychologic evaluation, monitoring of the patient's condition, health promotion, and prevention coordination of services, teaching and training activities, and direct nursing care.

Key Responsibilities:

  • Coordinate total patient care, ensuring seamless delivery of services.
  • Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis, identifying areas for improvement.
  • Prepare and present patient records to the Clinical Record Review Committee as indicated, ensuring accuracy and completeness.
  • Submit clinical notes and other record forms outlining services rendered, as required.
  • Participate in patient discharge planning, ensuring a smooth transition to post-acute care.
  • Perform other duties as assigned by management.

Requirements:

  • Graduate from an accredited School of Nursing.
  • Licensed as a Registered Nurse in the State.
  • One or more years of experience in community/home health agency or hospital preferred.
  • Knowledge of Medicare and Medicaid guidelines.
  • Criminal background check.
  • Current CPR certification.

Work Environment:

  • Must have a driver's license and be willing and able to drive to patient residences.
  • Ability to access patients' homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care.
  • Physical activities will include walking, sitting, stooping, and standing, with minimal to maximum lifting of patients and turning of patients.
