Position Overview

This role requires working closely with clients to provide them with the support and care they need on a daily basis under the direction of the Program Manager. The responsibilities include assisting clients with daily care, shopping, and transportation, documenting behaviors and medical intake, and ensuring the physical safety and wellbeing of supervised clients.

Key Responsibilities
  • Assist clients with daily care activities such as bathing, grooming, dressing, and eating.
  • Document medication intake and ensure clients are taking their medications as prescribed.
  • Assist with shopping and meal planning to ensure clients have access to healthy food options.
  • Provide transportation to appointments and community activities.
  • Monitor clients for any changes in behavior or medical conditions and report any concerns to medical staff.
  • Engage clients in therapeutic activities such as art, music, and exercise.
  • Help clients develop and maintain daily routines and schedules to promote stability and well-being.
  • Maintain confidentiality and adhere to HIPAA regulations at all times.

Candidate Qualifications and Experience
  • High School Diploma or Equivalent.
  • Previous experience working with individuals in behavioral health preferred.
  • Ability to work independently and as part of a team.
  • Ability to maintain confidentiality, professionalism, and customer service in all interactions.
  • Excellent communication skills and ability to establish rapport with clients.
  • Successful completion of HCBH pre-employment screening and background check.

Work EnvironmentThe employee is required to walk, stand, sit, and use the hands. Occasionally stoop, crouch, or kneel. The employee must occasionally exert or lift up to 25 pounds.
