The successful candidate will support five clinics in the south metro and three regional areas, with a schedule created four weeks in advance. This full-time position offers 8-hour shifts, no weekends, and mileage reimbursement.


Job Description:

As a Physical Therapist, you will be responsible for:

  • Providing therapy to patients and assessing their needs.
  • Gathering data and making clinical judgments to generate accurate therapy diagnoses.
  • Designing, implementing, and modifying therapeutic interventions.
  • Coordinating, communicating, documenting, and billing per program, regulatory, and professional standards.
  • Accommodating the selection of tests and measures and therapeutic interventions to the age of the patient or client.
  • Making referrals to other practitioners as appropriate.
  • Selecting and delegating appropriate tasks of the plan of care per professional guidelines.
  • Supervising the therapeutic program and maintaining oversight of the plan of care.
  • Promoting rehab services by ensuring patient and physician satisfaction.
  • Participating in performance improvement and process improvement activities for the department.
  • Supporting and promoting new services as requested.
  • Providing ongoing input to enhance the services provided by the department.
  • Developing and promoting new and current programs as requested.
  • Assisting co-workers in managing the teams' productivity/performance targets.
  • Managing their daily patient schedule.
  • Assisting in maintaining an efficient clinic flow, including assisting with scheduling, cleaning rooms, and laundry.

Job Requirements:

To be successful in this role, you will need:

  • A Bachelor's degree in Physical Therapy from an accredited program.
  • 0-2 years of experience in the practice or program area.
  • Licensed Physical Therapist - MN State Board of Physical Therapy (if providing services in Minnesota) or WI Dept of Safety & Professional Services (if providing services in Wisconsin).
  • BLS Tier - Basic Life Support - Multisource within 60 days.
  • A valid Driver's License (MN and WI residents must obtain a valid driver's license in their state of residence within 60 days of employment if providing services for Home Care and Hospice).

Physical Demands:

This role requires:

  • Medium Work*: Lifting weight Up to 32 lbs. occasionally, Up to 25 lbs. frequently.
