About the Role:

We are seeking a dedicated and compassionate Disability Advocate to join our team at Service Coordination, Inc. As a key member of our organization, you will play a vital role in providing exceptional support services to individuals with disabilities.

Responsibilities:

  • Advocate for individuals with disabilities, providing education and resource information to empower them towards self-determination.\
  • Collect information and documentation related to eligibility for DDA services and recommend eligibility and priority.\
  • Plan for, facilitate, develop, and continually update Person-Centered Plans (PCPs) that document service needs for individuals eligible for DDA services.\
  • Research, identify, refer, and coordinate resources and services to achieve the outcomes specified in individual's PCPs.\
  • Monitor Person-Centered Plans (PCP) and conduct follow-up activities.\
  • Adhere to SCI and policies and procedures.\
  • Collaborate with other team members to meet shared objectives.\
  • Communicate effectively with diverse groups of people.\
  • Build strong customer relationships and deliver customer-centric solutions.\
  • Ensure accountability and transparency in all interactions.\
  • Serve as a trusted advocate and advisor to individuals with disabilities and their families.\
  • Develop and maintain strong relationships with community partners and stakeholders.\
  • Maintain accurate and up-to-date records of individual progress and service delivery.\
  • Conduct regular assessments and evaluations to ensure optimal service delivery.\
  • Collaborate with interdisciplinary teams to provide comprehensive support services.\
  • Stay current with industry trends, best practices, and regulatory requirements.\
  • Participate in ongoing training and professional development opportunities.\
  • Meet performance utilization benchmark to ensure compliance with SCI standards.\
  • Maintain individuals' HIPAA records in secure locations.\
  • Utilize tracking and time management tools to optimize service delivery.\
  • Oversee provision of documentation to apply for medical assistance.\
  • Communicate effectively with providers regarding resolution of individual's concerns.\
  • Assist individuals with transitioning to new environments or situations.\
  • Provide education on how to coordinate and advocate for services.\
  • Educate individuals on the principles of self-determination.\
  • Support individuals in achieving their goals and aspirations.\
  • Encourage independence and empowerment among individuals with disabilities.\
  • Foster positive working relationships through a solution-based approach.\
  • Routinely explore options, areas of interest, and preferences for each person served.\
  • Establish and monitor an annual Person-Centered Plan that identifies outcomes important to the person.\
  • Secure ongoing support services and continuously monitors and evaluates the quality of the services provided.\
  • Meet state-mandated requirements for all duties of the Service Coordinator role.\
  • Identify and secure ongoing support services and provide assistance to individuals with developmental disabilities.\
  • Ensure that the services provided meet state-mandated standards.\
  • Promote a culture of inclusivity, respect, and empathy within the organization.\
