About the Role

We are seeking a highly skilled Behavioral Health Practitioner to join our team at Unicare Community Health Center. This is an exciting opportunity to make a meaningful difference in the lives of our patients and contribute to the growth and development of our organization.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. A strong understanding of behavioral health principles and practices is essential, as well as the ability to provide high-quality patient care and services.

This full-time position offers a competitive salary range of $**85,000 - $110,000 per year**, depending on experience, as well as a comprehensive benefits package and opportunities for professional growth and development.

Job Summary:

  • Provide behavioral health services to patients and families, including assessment, counseling, and advocacy.
  • Collaborate with healthcare professionals to develop and implement patient care plans.
  • Conduct home visits and outreach activities to promote health education and disease prevention.
  • Participate in interdisciplinary teams to coordinate patient care and services.

Requirements:

  • Masters degree in Social Work (MSW) from an accredited program.
  • Current licensure as an Associate Clinical Social Worker (ACSW) or equivalent.
  • Minimum 2 years of experience in behavioral health, preferably in an outpatient setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Bilingual English/Spanish proficiency strongly preferred.

Working Conditions:

  • Full-time position, Monday-Friday, 8am-5pm, with occasional evening or weekend hours.
  • May be required to travel to other Unicare clinics or community-based programs.
  • Must have valid CA driver's license and reliable transportation.
