Job Summary

The Licensed Clinical Social Worker (SW) will provide psychosocial and clinical support services to older adult patients and their families in the Geriatric Emergency Department. This role requires a strong understanding of the unique needs and standards of care for older adults in the ED setting.

Key Responsibilities
  • Conduct initial and ongoing comprehensive psychosocial assessments and develop care plans with patients and their family members.
  • Act as a liaison between internal medicine, geriatrics, and palliative care teams.
  • Provide mental health counseling and therapy, crisis intervention counseling, and grief counseling as needed.
  • Maintain accurate and timely documentation in the electronic health record.
  • Collaborate with community services to enable patients to resolve non-medical problems and facilitate referrals to other sources of assistance.
Requirements
  • Masters Degree in Social Work and current licensure in clinical social work (LCSW), LGSW or LICSW preferred.
  • Minimum of 2 years of clinical social work experience.
  • Knowledge of mental health resources available in Washington, DC preferred.
