We are seeking a skilled Clinical Care Coordinator to join our team at Penn State Health. As a key member of our palliative care team, you will play a vital role in coordinating the compassionate and effective care for patients with serious illnesses.

About the Role:
  • Job Type: Full-time
  • Shift: Monday through Friday, Day Shift
  • Employment Type: Staff

In this critical role, you will collaborate with referring providers, interdisciplinary teams, and community resources to address patient and caregiver needs in the ambulatory setting. Your primary responsibility will be to coordinate the care of patients with complex medical conditions, ensuring that they receive the highest quality of care possible.

Responsibilities:
  • Coordinate the care of patients with serious illnesses, including those with terminal diagnoses
  • Collaborate with healthcare teams to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
  • Identify and address potential barriers to care, including access to healthcare resources and social determinants of health
  • Provide education and support to patients and families on palliative care principles and options
Requirements:
  • Bachelor's degree in nursing required
  • Three years of nursing experience required
  • Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact required
