About the Role:

As a Nurse Navigator, you will play a critical role in ensuring seamless patient care coordination throughout their journey. You will work closely with our interdisciplinary team to identify patients' needs and develop personalized plans to facilitate their transition through the healthcare system.

Responsibilities:

  • Provide expert nursing guidance and support to patients and families
  • Coordinate care transitions between hospital and outpatient settings
  • Develop and implement patient-centered care plans
  • Collaborate with healthcare teams to ensure effective communication and coordination
  • Maintain accurate and up-to-date patient records

Requirements:

  • Bachelor's degree in Nursing (BSN)
  • Minimum 5 years of experience in direct patient care
  • Current licensure as a Registered Nurse (RN) in Pennsylvania or multistate RN license
  • AHA BLS certification
