Job Title: Staff Nurse I

VNS Health is seeking a skilled and compassionate Registered Nurse to join our team as a Staff Nurse I. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and promoting a culture of excellence within our organization.

Job Summary:

The Staff Nurse I will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with interdisciplinary teams to ensure seamless care transitions. This role requires strong clinical skills, excellent communication and interpersonal skills, and a commitment to delivering patient-centered care.

Key Responsibilities:
  • Conduct comprehensive assessments of patients to identify their unique needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and effective communication.
  • Develop and implement care plans that address patients' physical, emotional, and social needs.
  • Provide direct patient care, including administering medications, performing treatments, and providing education and support to patients and their families.
  • Monitor patients' progress, adjust care plans as needed, and communicate effectively with patients, families, and healthcare teams.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in patient care.
Requirements:
  • Current license to practice as a Registered Professional Nurse in New York State.
  • Minimum two years of experience in acute inpatient hospital experience in chronic or complex care.
  • Strong clinical skills, excellent communication and interpersonal skills, and a commitment to delivering patient-centered care.
  • Knowledge of Medicare and Medicaid regulations, as well as Milliman criteria (MCG).
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
