About the Role:

This is a field-based position that requires working with Medicaid members to ensure they receive the appropriate care. You'll conduct home and social assessments, provide member education, and address social determinants of health such as transportation, housing, and food access.

Responsibilities:
  • Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate level of care.
  • Utilize company and community-based resources to establish a safe and effective case management plan for members.
  • Collaborate with patients, families, and healthcare providers to ensure quality coordinated care and services.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Communicate with stakeholders to ensure required health-related information is shared to provide quality care and services.
  • Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
  • Document all member assessments, care plans, and referrals provided.
Requirements:
  • Bachelor's Degree in a health-related field or social work, or Active LPN license.
  • 1+ years of field-based experience.
  • 1+ years of knowledge of community resources, culture, and values.
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams.
  • Valid driver's license, current automobile insurance, and access to reliable transportation.
  • Ability to travel locally, up to 100 miles round trip, and up to 50% of the time.
Preferred Qualifications:
  • Community Health Worker (CHW) Accreditation.
  • Experience working in Managed Care.
  • Knowledge of Medicaid/Medicare population.
