RN Home Health Clinical Manager Job Description

At Interim HealthCare, we're dedicated to providing exceptional care to our clients in the comfort of their own homes. As a RN Home Health Clinical Manager, you'll play a vital role in overseeing our team of healthcare professionals and ensuring that our patients receive the highest quality care.

Key Responsibilities:
  • Oversee the management of patient care services, including patient assignments, coordinator patient care, and referrals.
  • Develop and implement individualized plans of care for patients, ensuring their needs are continually assessed and met.
  • Lead our quality assurance and performance (QAPI) program, focusing on patient outcomes and experience.
  • Ensure compliance with federal, state, and local laws and regulations, as well as Interim HealthCare policies and procedures.
Requirements:
  • Minimum of three (3) years of home health care experience required.
  • Strong leadership and management skills, with the ability to motivate and guide a team.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
