Job Title: LPN Medication Administration Specialist

Job Summary:

PROMESA R.H.C.F. is seeking a highly skilled LPN Medication Administration Specialist to join our team. As a key member of our healthcare staff, you will be responsible for overseeing the self-administration of medication, monitoring client health, and collaborating with program staff to ensure the highest level of patient care.

Key Responsibilities:

  • Medication Administration Oversight: Facilitate the monitoring of client's self-administration of medication, ensuring accurate and timely administration.
  • Medication Documentation: Appropriately document all medication taken by residents, maintaining accurate and up-to-date records.
  • Adverse Reaction Monitoring: Monitor clients for any adverse reactions to medications and take appropriate actions, reporting as necessary to ensure client safety.
  • Medication Delivery and Reconciliation: Accept and document deliveries of medication, perform daily medication reconciliations to verify counts of all clients' medication, and ensure compliance with all medication policies and procedures.
  • Controlled Medication Management: Secure, monitor, and distribute controlled medications, in compliance with all agency, city, state, and DEA regulations.
  • Collaboration and Communication: Coordinate with other nurses and staff to coordinate patient care and medication, document and share all non-compliance with medication regimens, and protect client confidentiality except in cases to coordinate care with staff.
  • Medication Education and Compliance: Conduct groups with residents to promote medication education and medication compliance, and coordinate processes to improve/evaluate medication administration, making recommendations as needed.
  • Client Referral and Coordination: Assist with referring clients to appropriate health care providers/specialists, coordinate with the client's primary care physicians to inform decisions regarding management and treatment of problems identified, and provide care as prescribed by a physician.
  • Prescription Management: Assist with the management of client prescriptions with medical providers and various pharmacies, with focus on Evers Pharmacy, and provide oversight and monitoring to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.
  • Documentation and Reporting: Oversee compliance with rules for documentation of medication administration, identify when a person's response to a medication or change in condition requires medical review and possible intervention, notify medical providers of this observation, and consult with Medical Director/Provider if a self-directed individual appears to no longer be capable of taking or understanding their medication.
  • Staff Collaboration and Communication: Collaborate and coordinate with 819 provider staff and provide reports as requested, participate in client clinical review meetings and coordination of care meetings as requested, and monitor and document clients' vital signs and additional vitals as needed to ensure client safety.
  • Medication Error Investigation: Promptly investigate medication errors based on severity index/potential severity in order to improve staff performance and patient care, work with Medical Director to identify and when required, to report errors to proper authorities.
  • Policy and Practice Improvement: Participate in meetings with staff and vendors that work to improve policies and practices, serve as a resource person by consulting with agency staff, and perform other tasks as assigned by Supervisor/s.

Requirements:

  • Education and Licensure: Registered Nurse from an accredited Nursing school with appropriate educational degrees, and a minimum of one (1) year of experience as an LPN.
  • License and Certification: Must possess an active and unrestricted license as a Licensed Practical Nurse (LPN) in New York State, and must possess current Basic Life Support (BLS) certification, and must possess current NYS approved Infection Control certification (4.0 hours, valid for 4 years).
  • Knowledge and Skills: Knowledge of substance/alcohol dependence/use population, must have specific training in chemical use, abuse, and dependence specific to the services provided, and ability to utilize and operate electronic record systems.
  • Training and Certification: Obtain within six months of hire: Supporting Recovery with Medications Addiction Treatment (MAT), CASAC Canon of Ethics (6 hours), Confidentiality related to 42CFR (3 hours), and Mandated Reporter (2hours).
