We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Navigator - Advanced Practice Nurse.

Salary: $115,000 - $150,000 per year, depending on experience.

About the Role:

The successful candidate will be responsible for providing safe, continuous quality patient care in a variety of settings. As part of a market-based nursing resource team, you will be assigned to varying units during your shift to ensure that patient care needs are met within the assigned facility.

This role requires experienced, proficient nurses within their specialty (medical/surgical, telemetry, critical/intermediate care, behavioral health, emergency or women's services). You will demonstrate independence, rule consciousness, social boldness, and change readiness.

You will exhibit a strong commitment to professional nursing standards and inter-professional collaboration. These nurses are early adopters of Novant Health systems, technology, and workflows. They serve as reliable consultants and mentors to clinical nurses and share lessons across teams to advance the practice of nursing throughout Novant Health.

Responsibilities:

  • Serve as a leader of the healthcare team by establishing an authentic personalized relationship with the patient and their chosen support system.
  • Collaborate with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health or supports a peaceful death.
  • Utilize best scientific evidence and compassion to assist the patient in navigating their health journey.

Requirements:

  • Education: 2 Year / Associate Degree, required. 4 Year / Bachelors Degree, BSN preferred.
  • Experience: One year experience Consecutive year within defined specialty (Med Surg, Tele, Critical Care, ED or Women's) required.
  • Licensure/Certification: Current compact licensure in appropriate state required. Specialty Certification preferred.
  • Additonal Skills (required): Appropriate customer service, effective and appropriate customer relations, verbal and non-verbal communication techniques, interpersonal relationship skills, conflict resolution, critical thinking and computer literacy.
