Key Responsibilities

• Collaborate with multidisciplinary teams to develop and implement comprehensive care plans for residents.

• Work closely with specialty clinicians to ensure continuity of care and promote optimal resident outcomes.

Requirements

• Master's degree in Social Work.

• Current licensure as an LISW in the state of employment.
