The Licensed Practical Nurse is a vital member of our hospice care team, providing direct patient care under the supervision of a registered nurse. This role requires a strong commitment to delivering compassionate and high-quality care to individuals and their families during a challenging time.
Key Responsibilities:- Provide direct patient care in accordance with the patient's plan of care and the LPN's scope of practice, as delegated by the supervising RN.
- Interview patients and record their medical history and physical condition.
- Obtain patient vital signs, including pulse, blood pressure, temperature, and respiration.
- Communicate with patients, caregivers, and their families to ensure seamless care coordination.
- Assist the registered nurse and provide care for patients as outlined in the plan of care, including administering medications, providing wound care, and preparing and administering injections and/or enemas.
- Implement current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient services in the electronic medical record.
- Graduate of an accredited practical nurse or vocational nursing program.
- Three (3) years' nursing experience, preferably in community health/hospice or medical/surgical settings.
- Currently licensed as an LPN in the State of Michigan and maintain such licensure in accordance with applicable laws and regulations.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
The physical demands of this position include vision, effective speech, and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in a motor vehicle for extended periods; standing, sitting, walking, bending, reaching, and stretching; lifting up to fifty (50) pounds unassisted and the ability to assist in lifting patients using appropriate lifting techniques and/or devices.
Proof of current tuberculin testing is required. Patient contact will not be allowed until tuberculin clearance is documented.
