- Document Patient Progress: Accurately document patient assessments, care plans, and outcomes to ensure continuity of care.
- Stay Up-to-Date with Industry Developments: Participate in ongoing education and training to maintain knowledge of best practices and industry standards.
- Education and Licensure: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing and current, unrestricted state license as a Registered Nurse.
- Experience and Skills: One year of experience in a clinical care setting, with a strong focus on patient care coordination and communication.
- Additional Requirements: Current CPR certification and ability to travel to patient homes.
- Home Health Experience: Previous experience in home health care, with a focus on patient care coordination and communication.
- Strong Communication Skills: Excellent verbal and written communication skills, with the ability to effectively communicate with patients, families, and healthcare teams.
- Physical Demands: Ability to lift 50 pounds, stand for prolonged periods, and travel to patient homes.
- Environmental Demands: Ability to work in a variety of environments, including patient homes, and exposure to allergens and infectious diseases.
