ResponsibilitiesClinical Excellence & Patient Care
  • Possesses the knowledge and clinical skills necessary to provide care for individuals with physical disabilities and chronic illnesses.
  • Participates in patient/family educational activities and uses appropriate resources to implement an individualized teaching and discharge plan.
  • Performs hands-on nursing care by utilizing the nursing process to achieve quality outcomes for patients.
  • Reinforces the care provided by other team members.
  • Provides direction and supervision of unlicensed assistive personnel.
  • Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team.
  • Demonstrates effective oral and written communication skills to develop a rapport with patients, their families, and health team members.
  • Maintains a strong commitment to the multidisciplinary health care team.
  • Contributes to the assessment of the physical, psychological, sociocultural, and spiritual dimensions of patients and their families.
  • Collaborates with other rehabilitation team members and contributes to the dynamic interdisciplinary plan of care.
  • Implements the interdisciplinary plan of care within the scope of LPN/LVN practice.
  • Contributes to the evaluation of the nursing care and to the modification of the plan as needed.
  • Contributes to the interdisciplinary team process at team conferences and other team meetings.
  • Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient.
  • Collaborates with the staff nurse and other team members to achieve cost-effective care.
  • Communicates effectively and interacts in a manner that facilitates the group process and team building.
  • Advocates and supports the rights of patients and families with care decisions.
Clinical Excellence & Patient Education
  • Reinforces information provided to patients about the disease processes underlying disabilities and teaches nursing techniques to help patients and their families develop the self-care skills necessary to move toward wellness.
  • Provides assistance to patients and their families within his/her scope of practice for future self-management and decision-making responsibilities.
  • Reinforces the teaching done by the staff nurse and other specialists and other healthcare disciplines.
  • Demonstrates knowledge that patient and workplace safety is a priority.
QualificationsEducation

Successful completion of a Licensed Practical Nurse program from an accredited college.

Experience

Experience in Post-Acute Care or an Acute Care environment.

License(s)/Certification(s)

Current licensure in good standing to practice as a Licensed Practical Nurse within the state you are practicing.

Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
