As a Licensed Vocational Nurse (LVN) in our hospice team, your responsibilities will include:
- Adhering to a personalized care plan based on the individual needs of the patient.
- Executing assigned tasks such as medication administration, wound care, and other necessary treatments.
- Following up on and accurately documenting physician orders.
- Maintaining thorough records of observations, interventions, and evaluations related to patient care.
Qualifications:
- Valid Practical/Vocational Nursing License in the relevant state.
- Preferred minimum of one year of nursing experience.
- Experience in home health or hospice care is advantageous.
- Current CPR certification.
- Excellent organizational and communication skills.
- Proof of current auto insurance.
- Documentation of Covid vaccination, medical exemption, or religious exemption.
